2024 Ulmstead Swim Team Registration and Volunteer Selection

We are excited that you have chosen to swim with the Ulmstead Ducks this Summer 2024!  IF YOU HAVE ALREADY REGISTERED THIS SEASON, PLEASE DO _NOT_ REGISTER AGAIN.  IF YOU NEED HELP ACCESSING SOMETHING OR IT DOESN'T LOOK LIKE YOUR KIDS ARE ALREADY REGISTERED, PLEASE EMAIL [email protected].

PLEASE DO NOT SIGN UP IF YOU CANNOT COMMIT TO THE REQUIRED VOLUNTEER ASSIGNMENTS.  (If there are special circumstances that mean you cannot volunteer as required, you MUST reach out to the Board BEFORE registering.)  Registration fees remain the same this year ($150 for the first swimmer in your family and $140 for subsequent swimmers).  If you choose to withdraw your registration for a swimmer, the registration fee minus credit card processing fees is refundable PRIOR to the first day of practice. There are no refunds available beginning the first day of practice, May 28.  A late registration fee of $50 per family will apply on 5/25.

Volunteer Shifts:  Each family is responsible for 4 points (the equivalent of two full meets) worth of volunteering.

Swim Caps: If you already registered and forgot to order swim caps during registration, please go to The Team Store.  Personalized caps must be ordered by May 1.  After that date, the standard team cap will still be available for purchase as long as supplies last.  Our generous sponsors allow us to provide a team t-shirt to each swimmer who registers by May 15.

Reach out to us if we can answer any questions!

Minimum Swimming Ability

I understand that swim team is different than swim lessons, and I attest that all swimmers I register today can swim the length of a 25-yard pool unassisted.

Liability waiver

I, on behalf of the parents or legal guardians (hereafter referred to in the singular) of the above registered swimmers (hereafter referred to as the “child”), request that the child be allowed to participate with the Ulmstead Swim Team program (hereafter referred to as “the activities”). This agreement shall remain in effect until Ulmstead Swim Team receives written notice of the cancellation of the consent or until the end of the activities described above. In return for the child being permitted to take part in the activities and to use of the facilities, property, and equipment of Ulmstead Swim Team and Ulmstead Swim Club (whether by written agreement or informal arrangement), each of us makes the following promises and warrants the truth of the following facts: 1. I am familiar with the programs included in the activities, and I understand officers and employees of Ulmstead Swim Team are available to discuss the activities if I should wish additional information. I also understand that I am solely responsible for the arrival and departure of my child at the beginning and end of each day’s program. I will not allow my child to remain on the premises of Ulmstead Swim Club after each day’s program without appropriate supervision. I agree that Ulmstead Swim Team will have no responsibility for the supervision of my child at times other than during the scheduled activities. I will inform my child that he/she is expected to cooperate with, and follow the directions of, the persons in charge of the activities and to act in a manner consistent with the spirit of good sportsmanship and respect for the rights of others. 2. My child is in good health, and I know of no reason why he/she would be incapable of participating in the activities. My child knows how to swim. I will immediately notify the coach and/or assistant coaches of the Ulmstead Swim Team if a change in my child’s health or other condition would affect my child’s ability to participate in the activities. 3. WAIVER OF LIABILITY: I waive and release any right I, my heirs, distributees, guardians, legal representatives and assigns may have or acquire to make a claim against, sue, attach the property of or prosecute Ulmstead Swim Team or any of its members, directors, officers, agents, employees and affiliated organizations (hereafter referred to as “the releasees”) for monetary damages caused by injury to my child or damage to the property of my child or myself arising from my child’s participation in the activities and use of the facilities, property, and equipment of Ulmstead Swim Team and Ulmstead Swim Club (whether by written agreement or informal arrangement), whether or not the injury or damage results from the negligence or other action, except intentional acts, of any of the releases. 4. ASSUMPTION OF RISK: I accept any and all risks to myself and my child of injury, death and property damage arising from participation in the activities and the use of the facilities, property or equipment of Ulmstead Swim Team and Ulmstead Swim Club (whether by written agreement or informal arrangement), whether or not caused by the negligence of other action, except intentional acts, of any of the Releasees. 5. INDEMNITY AGREEMENT: I agree to indemnify and hold the releasees harmless from any loss, liability, damage or cost, including reasonable attorneys fees, they may incur due to my child’s participation in the activities and use of the facilities, property and equipment of Ulmstead Swim Team and Ulmstead Swim Club (whether by written agreement or informal arrangement) whether or not such a loss, liability, damage or cost results from the negligence or other action, except intentional acts, of any of the releases. I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THE AGREEMENT INCLUDES A WAIVER OF LIABILITY. AN ASSUMPTION OF RISK AND AN AGREEMENT BY ME TO INDEMNIFY THE RELEASEES, AND I SIGN IT OF MY OWN FREE WILL.

I, on behalf of myself, my child, and any other parent of my child, voluntarily wish to assist or participate in events or activities sponsored by the Ulmstead Swim Team. I represent and warrant to Ulmstead Swim Team that I have authority on behalf of any other parent of my child to represent, warrant, waive, agree, etc. as set forth herein.

I understand that I, my child, and any other parent of my child do so at our own risk and I, my child, and any other parent of my child hereby waive and release Ulmstead Swim Team, its volunteers, members, agents, servants, employees, or representatives and any other organizing or sponsoring organization (collectively “UST”) from all claims and liabilities of any kind arising from or related to my or my child’s participation, including but not limited to illness, bodily injury, death, property damage, liabilities, losses, damages, fines, costs, fees, and expenses including attorney’s fees and costs (at both any trial and appellate levels) arising from or related to this waiver or me or my child’s assistance or participation as contemplated herein (collectively “Claims”). I, my child, and any other parent of my child hereby agree to waive all rights of subrogation for any Claims against UST. I hereby agree to defend, indemnify, and hold harmless UST from any Claims against it by any person related to my or my child’s participation in any UST event or activity. I understand that I am responsible for any insurance needs in case of any illness, harm or injury and that no insurance is provided except for that offered by the Amateur Athletic Union of the U.S., Inc. UST is responsible for purchasing this policy for each swimmer prior to their participation in the activities.

Furthermore, I understand that although safety precautions will be observed, UST will not be responsible for any Claim arising from or related to my or my child’s participation, or for any illness, harm or personal injury I or my child may sustain in participation at any UST event or activity.

I also consent to UST’s use of any photographs, recordings, data, information and/or videotapes made of any event or activity, whether through an online internet or virtual presentation or provided in person and I, on behalf of myself, my child, and any other parent of my child, waive any privacy, ownership or copyright concern in any presentation or recording of me, my child, or any other parent of my child at any event or activity, on the internet online or in a virtual setting.

● Ejections occurring at the end of the season will result in the suspensions being carried over to the next season. The suspended individual may be barred from all sites where league-sponsored activities are being held.

A coach or league representative who refuses to provide the full name of an ejected individual to league or meet officials will be subject to disciplinary action.
